This has happened to me more often than other failures & always the grommet is of poor design & too stiff as is the cable it should protect.
Then of course users tend to apply strain to their cables without thinking.
You were lucky, generally I find that the shorted cables at the failure point cause other damage which makes any repair uneconomic.
Replacements from Ebay are generally low cost.
